1972

 

Park Hill Family Health Center

In early 1972 Furman Knox (director of the Louisville CAC) and Dr. Maurice Fliegelman (dermatologist associated with Jewish Hospital) developed idea of a neighborhood health center to serve the California-Park Hill Neighborhood and selected Betty Fleishaker to research the project. A VISTA volunteer who worked with Mrs. Fleishaker and Susan Charles submitted and received a grant under the Federal Community Health Program to start up a community facility named initially the Park Hill Family Health Center (later changed to HealthCare of Louisville.) Congress never officially authorized the Community Health Program which issued grants for start up health centers, but by the time it scuttled the program the 42 grants (including that to Park Hill FHC) were allowed to continue. 28 of the 42 health centers survived during the initial grant period.

St. George\\\\\'s Catholic Church convent at 1809 Standard Ave. was remodeled as medical facility. Bob Evans supervised much of construction.
With the advent of the Federal HMO Act of 1973, the Board of Directors received approval as a pre-paid staff model HMO and changed the name to HealthCare of Louisville, Inc.

 In the early days the Board of Directors included Betty Fleishaker, Paul Schweitzer, Frank Doheny, Dee Dee Jones, Don Shelton, John Blalock, Ted Winfield, Richard Miller, Mattie Hart, Larry Kries, June Fussenegger, Dan Klein among others. There was also a Member Council.

In March 1981 the Board of Directors of HealthCare of Louisville, Inc.. approved a management agreement with Healthplans Management Corporation of Nashville. Under the agreement, Healthplans assumed responsibility for the administrative aspects of the HMO, while HealthCare remained responsible for the delivery of health care benefits and services. There was also an option for Healthplans to purchase HealthCare in the future. Healthplans of Nashville also contracted to manage a group practice in Lexington and changed its name to Health Plans of Kentucky. Within a couple of years Healthplans made other acquisitions and changed its name to HealthAmerica. [In April 1984 HealthAmerica exercised its option and purchased HealthCare of Louisville outright.]
The new Healthplans management created a new logo of HealthCare in Dec \\\\\'81.

In April Dr. Adams moved to Broadway full-time and transferred her Pediatic practice at Greentree to Broadway.

In May Dr. Bratton rejoined the Broadway group and closed her previous private practice at 31st and Broadway.

In June Dr. Miller left Newburg and moved to Columbus, OH.

By the end of July Drs. Joshi, Inbaraj, Metz-Dunn, Minter-Sauer and Watters left the practice resulting in the closure of the Greentree and Middletown offices.  Dr. Joshi opened his own solo practice in IN and Drs. Inbaraj, Metz-Dunn, Minter-Sauer and Watters left to join a CMA group.

Effective December 31, 2004, Drs. Box, Newstadt, Tanamachi and Witherington vacated the Newburg office and joined CMA while Drs. Edwards and Samples joined Caritas which took over operation of the Southwest office.

Cypress Medical Associates began 2006 with four physicians, a physician assistant and about fourteen support staff who provided medical services in the two offices listed below. Elisa Gentry, a nurse practitioner, joined Dr. Remson at the Newburg office. In July, 2005, the group learned that Humana sold the 2650 Broadway building to Seven Counties who planned to occupy the entire facility at the end of Cypress lease Dec 31, 2006. By August 2006 the group had selected the 2500 Building as its next location and began the process of modifying the old Quiznos space. In early December 2006 the group learned that Seven Counties required Cypress to clear out the basement as well. The basement contained about 12,000 boxes of charts from many of the previous centers as well as Bluegrass in Lexington. Through heroic efforts all these boxes were moved, mostly to the second floor and basement of the new 2500 Building. At the beginning of 2006, Dr. Remson moved to offices on the first floor of Newburg. Humana sold the Newburg to the Watterson Group in 2006.

For the first three months of 2007, the Broadway Cypress group worked in temporary quarters in the Lyles Mall. On March 26, the group began seeing patients on the first floor of the 2500 Building. Dr. Bratton left the practice in early June to work at Park Duvalle.

The owners of the Broadway office building began charging rent for second floor storage space, and many hours and thousands of dollars were spent purging old records (from the Middletown, Greentree, Preston, and Newburg offices) that had been moved some seven years ago from the basement of the old 2650 building.
The group was saddened by the deaths of Drs. Walker and Sharp during the year.

Cypress Medical Associates began 2016 with three physicians, one mid level provider and twelve support staff who provide medical services in the same two offices. At the end of 2016 Cypress build out loan was paid off and rather than exercise another five year lease from BMW, the office signed a one year lease with options for four more one year leases. Cypress ended the year using only about 90 sq ft of the second floor "rentable" space. The offices continue to use Care360 as its hybrid EMR and maintain their association with the QIP ACO. AT the end of the year Dr. Arnett resigned from RBA and gave his 6% share of BMW to Drs. Remson & Adams who continued to own 18% of the 2500 Building.

2017

Cypress Medical Associates began 2017 with three physicians, one and a half mid level providers and twelve support staff who provide medical services in the same two offices. The Broadway office signed a one year lease of the space from BMW with two additional one year lease options. The offices continued to use Care360 as the hybrid EMR and maintain an association with the QIP ACO. In the fall of 2016 Passport switched its reimbursement from capitatated to fee-for-service;  this resulted in a $100,000 shortfall in revenue made up by physicians loans to the practice.

Cypress Medical Associates began 2018 with three physicians, one mid level provider, and nine support staff who provide medical services in the same two offices. The Broadway office agreed to another one year lease of the space from BMW with an additional one year lease option next year. Dr. Remsons office moved to new quarters at Newburg in October and Bellarmine purchased the building. The offices contnued to use Care360 as the hybrid EMR and PBSI. The group maintained an association with the QIP ACO. During the year the group struggled financially facing aprox. 80% overhead and the resulting need for the three physicians to go without pay for several pay periods (Remson - 9, Adams and Arnett - 26 each). By year end all providers were working at reduced pay but were able to keep the practices going.

2019

Cypress Medical Associates began 2019 with three physicians, one mid level provider, and eight support staff who provided medical services in the same two offices. The Broadway office agreed to another one year lease of the 2500 Building space from BMW which ran until Feb 1, 2020. Dr. Remson continued to practice at the Newburg office renting from Bellarmine, the new owner of the building. The offices contnued to use Care360 as the hybrid EMR and PBSI for billing.  The group maintained an association with the QIP ACO.

At the end of 2018 some 700 boxes of old charts (from the old offices of Greentree, Middletown, Southwest, et al that had been stored in the basement of the 2650 building and then moved to the basement of the 2500 building) were moved to ground level and haulded off by Shred-It on Jan 2, 2019.  An additional 400 boxes (mostly of old Newburg pediatrics) were shredded in Feb, and 230 boxes of old Broadway adult charts were carried off in March.  The only remaining medical records at the 2500 Building were 170 old Broadway adult chart boxes and 120 old pediatric chart boxes plus about 10,000 "active" charts in the office. The charts not sent to Norton were shredded in Nov 2019 & Jan 2020.

In May Mick Vissing left the Cypress Broadway office and joined the St. Matthews Norton office working with Dr. Punita Rastogi Halder.  Norton  opened a primary care office in December at 17th & Broadway as part of the newly built West End YMCA; Mick initially planned to work at the new Norton office but chose later to stay at St. Matthews.   Dr. Adams retired from her Pediatric practice in the summer.  Dr. Arnett continued to practice at the Broadway office but most of the support staff found jobs elsewhere. Norton agreed to take the Pediatric and adult charts to their Iron Mountain storage facility..

2020

Cypress Medical Associates began 2020 with two physicians and three support staff who provided medical services in the same two offices at the start of the year. Dr. Arnett decided to leave Cypress and begin practice on a part time basis at the new Norton office at 17th and Broadway beginning in April 2020. His 800 or so active charts were tranfered to the Norton Iron Mountain storage facility. Laura who had joined the practice as an MA at the same time as Dr. Arnett in August 1978 agreed to stay on until the end of March and then retired. Anna began a new job Jan 6, and Ron left just before Christmas. Debbi went to work with Mick Vissing in the summer or 2019, Brigette joined Nortons at the new office on Broadway (and worked with Dr. Arnett for the remaineder of 2020), and Sheena joined the Norton Pediatric office on Broadway.

At the end of December, Dr. Arnett had an e-mail from Dr. Carl Wolf who was at Standard Ave when Arnett joined the practice over 41 years ago, and they enjoyed reminiscing. 

Dr. Duncan began at the new Norton office in December 2019, Drs. Giles and Crecelius began working there in January 2020, and Krista Garza, ANP, began in May 2020. Dr. Arnett began seeing patients just one day a week beginning in September and announced to many of his patients that he\\\\\'d be retiring at the end of the year.

In March the Covid-19 pandemic forced most offices to go to virtual operations.  Dr. Remson did this for several months working from home, and then decided to give up the Newburg office and Cypress, and on July 1 he began working for Baptist Healthcare Group on Hikes Lane.

Newburg: Remson until July 1, 2020 when he joined a Baptist Healthcare Group on Hikes Lane.  He folded his practice into the new office and stopped seeing patients at Newburg.
Broadway: Arnett until April 1,  2020 when the office closed.

 

2021

Cypress closed offices at Broadway in April 2020 and at Newburg in July 2020. The electric power was finally turned of at Broadway in Nov 2020. As of Feb 2021 the 2500 building remains empty except for Volunteers of America and a chiropracter office.  Dr. Arnett worked for Norton part time until the end of August 2021 when he retired after practicing medicine in Louisville for 43 yrs.  He graduated from UK Med School in May 1971, 50 years ago. Dr. Remson continues to work full time at the Hikes Lane Baptist Medical Group. Norton did not take the sixty boxes of records of those who had died, and Dr. Arnett is paying for these to be kept in a storage locker and each year for the next seven will gradually send the appropriate records to be shredded. Likewise old pediatic immunizaton records are being kept in storage per state requirement.  On October 19, 2021, Cypress Medical Associates was administratively dissolved.

 

2022

Cypress closed offices at Broadway in April 2020 and at Newburg in July 2020. Dr. Arnett worked for Norton part time until the end of August 2021 when he retired, Dr. Remson continues to work for the Baptist Medical Group.  The KY Secretary of State administratively dissolved Cypress Med Assoc on Oct 19, 2021.

Dr. Arnett continues to pay for the storage of old death charts (several more years to go) and immunization records. 

As of  Jan 1, 2022 there is still no replacement tennant in the old Cypress space at the 2500 Building which was built for the Universal Car Company in 2023.

Mick Vissing continues to practice with Dr. Halder at the St. Matthews Norton Medical office.  Dr. Adams remains in retirement. 

 

2023

Cypress Medical Associates officially went out of business Oct 19, 2021.

Dr. Arnett moved old death charts (several more years to go) and immunization records to a secure location at his home in spring of 2022.  Inventories of old record charts which were sent to Nortons are used periodically to respond to requests for old records. Dr. Arnett visited the Norton office at 1720 W. Broadway monthly with donuts and remained in contact with some of the old staff. In 2023 he happened to meet several of his old patients (Scheid, Madison, Heinz, Seiffertt, Tucker, et al).

The old Cypress space at the 2500 Building was taken over in 2022 by Amped (ampedlouisville.org) and the rest of the 2500 building by  JCPS. The former space was gutted and eventually is to be a performance area,  but remains unfinished as of Jan 1, 2023.   Dr. Arnett sent photos of the old Universal Car Show Room to the contractor responsible for developing the empty space.  2023 is the 100th anniversary of the contruction of the 2500 Building.

Dr. Remson contines to practice with the Baptist Group on Hikes Lane; Drs. Arnett and Adams remain in retirement; and Mick Vissing continues to practice with Dr. Halder at the St. Matthews Norton Medical office.

 

2024

** When Cypress Medical Associates ceased its operations at the Preston Office, Dressers group continued to rent space from Human for its practice (separate from Cypress). Dr. Chary continued a private adult practice (separate from Cypress) at the Preston office as well.
